You need to step out of your comfort level and do something that she would never think you would do, nothing crazy, but show her you can have a good time, and find away to get back to the times when the both of you loved each other

Maybe she is falling out of love with you but maybe it is just her getting bored with you, and starting to doubt your relationship. Try listening to her, so special things for her that she wouldnt expect. One day when she mentions something she likes to do, out of no where plan a day of doing that with her (even if its something you hate, but dont let her know you are hating it). Give her a small gift just because its "whatever day of the week". I think why this happens so much is because people get comfortable with each other and stop putting so much effort into the relationship, well now is the time to get uncomfortable and treat her like you just met her, do the things you used to when you first got together. and hey if none of this works, maybe she is actually out of love with you and its time to move on.

Love is not something you "fall out of or into." It's an action.
May I suggest communicate with her.
Maybe she needs some space?
Maybe it's time to move on?
Communication will take you a long way here. Be willing to hear even the bad news and take it graciously.
If it's time - part as friends and move on.

Its probably something you're not doing that made her fall out of love with you. Give her more attention and do something that shows your care for her, like call her more often, hug her, give her flowers, and etc. But you should talk to her and see why she is falling out of love with you. If she's falling out of love because she feels that theres something she wants that you dont have, you should move on.
