It refers to someone who has brain damage from blows to the head. It often describes boxers who have taken too many "punches". The damage causes them to lose coordination and their speech may be slurred. Outwardly they may appear to be intoxicated even if their mind is clear.

When you get punched a lot you get disoriented and start acting funny--like you are drunk. Maybe because your brain is swelling up. Punch Drunk.

punch drunk is a term for a boxer that has taken to many hits to the head and has effected him mentaly
