If you are playing an instrument, it is a mental process that you practice. As you play the notes you are also mentally counting them. If you are practicing by yourself you can use a metronome to play the beats. If you are playing in a group you usually have a director.

When you first start playing(or singing) you can use a foot tap to aid your mental counting. As you progress not only is it a mental thing, but it becomes a physical feeling of the beat pulsations.

This is a good question that really has no cut and dried answer. The road can be different for different people. If this doesn't help, please expand your question.
