What you describe sounds a lot like an iron deficency, which is easily treated. (A few weeks of iron supplements and you'd be bright awake again.) But like other people pointed out, there could be more serious reasons for your tiredness. So you better see a physician - and make sure they check your blood.

Firstly get yourself off to the doctors and get a physical check done. If you're not happy with the diagnosis get a second opinion after all, you've had these symptoms for as long as you can remember (i.e. Diabetes has these symptoms as well)

In general, maintaining a good stable diet and exercising can fix most problems i.e. eat foods that give you all the vitamins you need, walking 30 minutes a day is good exercise for the heart and the rest of the body

I know what you mean. If I go out to the mall for a while and come back, I am exhausted. A trip to the grocery store makes me tired too. I can seem to have a full day of activities because I need to lay down and rest
