My husband felt like something was rattling around inside his ear/head, so he shook his head like you would as if water was in it, when a blood clot fell out. Could over zealous q-tipping cause a blood clot?

Yes, it is common people perforating/rupturing their ear drums using q-tips. That is why the instructions tells you not to stick them in your ears.

The vast majority of rupured ear drums will heal on their own. If it does not heal on its own, it requires a graft to repair the ear drum.

What to do:

Clear out any debris from the canal.
Place a protective cotton plug inside of the ear canal and keep the canal dry.
Take an appropriate analgesic (e.g., ibuprofen, naproxen, acetaminophen with codeine or oxycodone).

What not to do:
Do not instill any fluid into the external canal or allow water to get into his ear. Water in the middle ear is painful, irritating and may introduce bacteria. Covering the cotton plug with petroleum jelly will allow the patient to shower safely.

Most likely he perforated his eardrum at some point. If that's what the clot came from, it's not an emergency.
