The "maker" of a check is the person that wrote the check.

'Return for maker' sounds like you forgot to sign the check but I don't know for sure.

I have no idea what code S might mean.

Perhaps the code is "Refer to Maker". This could mean a variety of things, but it usually means that the check had a stop payment on it.
