Funeral directors have a form that they fill out for such things. You should ask the funeral director to provide you with the appropriate form and then you provide it to the clerk of the court.

Did you sign the book of condolence as you left the chapel? That would be evidence, if necessary. You could get the family to photocopy the page.

Just get a friend to write a depo and have it notorized that you were there.
