I'd have to go for the Johnny Cash version of Hurt too. I can't think of a song I've heard that's sounded more heartfelt.
A close second would be Kanye West's Golddigger as covered by The Automatic just cos it's daft and good.

I love Me First and the Gimme Gimmes' cover of "99 Luftballoons" by... I can't remember who originally did it. The Ramones' cover of the Spiderman theme song is really good. Coheed and Cambria occasionally does a cover of Iron Maiden's song "The Trooper" at their shows, and it's awesome.
