If a patient is pleasantly confused, that means they are confused or disoriented, but they aren't agitated about it. They are probably easily reoriented or redirected, they probably maintain a smile on their face. They aren't lashing out or being mean as some confused people do. They may laugh about the mistakes they make, or be so oblivious to it that it doesn't cause caregivers a problem.

I didn't know there was a medical meaning to "pleasantly confused". Would this be a mild form of senility? If so, I don't see how that can be pleasant in any way....

This definitely has to be someone who is bi-polar. Happy and content one minute and confused and defensive the next. I personally know a couple people like this that are men. Does that count?

Urea is toxic. People with renal failure become less alert, more confused and slowly drift into coma. Usually, they are -"pleasantly confused" - but can be agitated. Rarely they become psychotic

It is just a phrase which relates to all kinds of dementia and not just one specific type.Medical staff use it when feeding back on patients and it has no sarcastic undertones implied.
