Originally the term was coined to describe those born with a gender that didn't match their physical form, and so took steps to correct the anomaly. Unfortunately it has now been hi-jacked by the transvestite and cross dressing community to try and legitemise their fetish.

It is an over arching term used to describe someone who crosses the boundaries of societal view points of gender. It is often used incorrectly to describe a transsexual. In reality, anyone who is a transsexual, crossdresser or Bi-Sexual would very easily qualify.

It's a broad term used for the entire community of gender-variant people. It includes crossdressers and transvestites, transsexuals of all kind from post-op to non-op, and also the other gender non-conformists that live beyond the labels male and female.

Transgender refers to someone who is physically born one sex, but mentally believes they are another. Some will choose to have an operation to reverse their physical sexuality. During this process they usually will begin taking testosterone or hormones to begin what is called "transitioning."

Simple really, it just means someone that changed sex from one sex to the other. Trans means to cross and gender I would have thought was pretty obvious, it means either male or female.
