Squirt a load of thick bleach in there, and round the rim. Leave for a while without flushing, then come back and scrub with a toilet brush.

If that doesn't work, try doing the same thing with one of the proprietary toilet cleaners with limescale remover (such as Harpic), but bleach will be a cheaper option.

Acid dissolves limescale, you can get tubs of citric acid from a chemist or homebrew shop. Dissolve a couple of tablespoons in 1/2 pint hot water, add some cold.
Get the loo brush and push the water in the loo round the u bend, then add the cool acid mixture and leave for a couple of hours. Repeat every night until its flaked off.
Warn anyone to put the lid down and flush before they use it!.
White vinegar would also work. Bleach will just bleach it white.

Pour a can of coke (the drink:-) in the toilet and leave over night. You'll be surprised how well it dissolves the limescale. Too finish off, do the same treatment the next day with a couple of Alka Seltzer tablets.
Voila.

You need to use a strong limescale remover...pour it down the loo...plenty of it, and do not flush the loo for 24 hours.

Repeat if necessary. For any stubborn lime scale, you'll have to chip away at it, then repeat the lime scale procedure.

Don't mix toilet cleaning products as the mixture can give off dangerous fumes. Stick to one at a time.
Good luck

Go to a plumbers shop and buy a bottle of 'Spirits of salt' this is very concentrated hydrochloric acid and pour that into loo. PLEASE MAKE SURE THERE IS NO BLEACH IN THERE AS THEY REACT TO GIVE OFF A NASTY GAS. It is best to wear eye protection and long heavy rubber gloves when doing this. Leave a few hours then flush.

I saw how clean is your house and they put on rubber gloves, then used a pummice stone on it and it came up lovely without damaging the enamel, they said you won't be able to remove it with a product you just chuck down the loo.
