First, document what the teacher is or is not doing.
Get your classmates to back you up. If you go alone, you might just sound like a whiner.
The person to go see is her department head. If you get no satisfaction, go to his boss and on up the ladder.
The important thing is to follow the chain of command. And to document--the best way to do that would be to tape record the class.
However, the law in Michigan and probably most states is that you can't record someone unless they know it. So, just put the recorder on your desk but kinda sort of hide it where it is in plain sight. Should be easy since they come pretty small now.

Transfer out! And if you are asked why, tell them. Most of the time getting a bad teacher out of a school is nearly impossible. If you are truely desperate to get something done about this horrid teacher, record a normal teaching day. Several of them. Then with their own words damning them, the school should have to do something. Make sure your parents are on board with this. Things could get really ugly before they get better. Which is why I said, to transfer out.

Report him/her to the school board. Include documentation and give specific examples.
I had a teacher call me stupid once when I asked a question. I told her, "No. are you?" She sent me to the principal's office. When I told the principal why I said what it did, you could hear the principal yelling at her down the hallway. She almost lost her job. I didn't know at the time, but I found out that I had the highest IQ in the school!
