Anyone knows where I can download patients' clinical data set for research purpose?
The dataset should have (but not limited to) the following information:
Patient Demographic Information
Medications/Therapy information
Symptoms, Reactions.
Results
...


The personal identifiers should be removed or anonymized.

Sorry mate, such information is not freely accessible and it's a violation of privacy legislation. You need to collect the data yourself by seeking hospital board ethics approval for going through hospital records. If you're looking specifically for drug reactions, speaking to a pharmacist may allow you access to their records, but you need to speak to your supervisor about this.
