Kind of a funny question... I like it.
My list:
1 - Volcano/Earthquake (heavy on the earthquake). Almost all the worst natural disters in history were earthquake events (except the worst -- the flooding of the Yellow River in the 1930's was the biggest killer ever, but most the other big ones were earthquakes). Volcanoes rarely result in a whole lot of deaths, but if a super volcano (like the one in Yellowstone Park) were to erupt it could result in global cooling and possibly billions of deaths.
2 - "Earth" Warming (usually called global warming or climate change). Whether or not humans are responsible, the planet is getting warmer, and this will have many unpleasant side effects. The melting of tropical glaciers will cause a huge problem in Eastern Asia and parts of South America as these are the source of most of their drinking water. Rising sea levels will cause billions of dollars of damage to infrastructure, and force the relocation of millions of people. Changing weather patterns will cause traditionally good farm-land to be useless. It isn't likely that a lot of people will be killed, but it will definately be a hardship if the planet continues to warm.
3 - Atom Bombs. I don't think it's likely that nuclear weapons will be used, but a nuclear war would definately have a very negative affect on the places where the weapons were used. Still, I think the chances of nuclear war are very low.
4 - Human Procreation. While this has created millions of people that I wish had never been born, human procreation is generaly a good thing.

You forgot to add Paris Hilton and American Idol as choices.
There are enough atom bombs to ruin all of our lives, but I doubt they will be used. Human procreation is likely to affect those people in the fastest growing countries first, which would affect the quality of life of many billions of people, but unfortunately for them their standard of life is already quite low comparatively (judging by sanitation, disease and longevity- I'm not judging their lifestyles). Global warming is a good candidate, and global cooling would also have a pretty huge impact on a large piece of the population. Earthquakes are pretty unlikely since they are localized events, but a large enough supervolcano could trigger world climate change, which is the same as global warming/cooling- so I'm going to go with global climate change. There are huge population centers in southeast Asia that will be uninhabitable in a hundred years or so, as well as low lying coastal areas on nearly every other continent.

I think it would be nuclear weapons. I can't help but to believe in that if one happens to hit a major city or two, a bunch more would follow (both for defensive and offensive purposes) and then the Human Race can try again.
