The truth is that food is food. Looking it at the situation from a logical standpoint, anybody who owns a restaurant wants the same high quality for every customer, so that they retain said customers. A bias in serving would lead to a dishonest organization and a decline in patrons as a cause. Plus, it's a pain in the butt to make one dish two ways :)

Certainly not. I've worked in the restaurant industry for so long now and I really have never seen anyone (the owner, the cook the managers) make a distinction between customers when it comes to food preparation. Now as for servers... well, let's just say that when you go out to eat, be nice to them because at some point THEY are in control of your food.

As a chef I would say no. Not to say that if you are in an establishment on a regular basis (that is you are a regular) that they wouldn't show some special treatment. One has to remember that in most cases the cooks can't see the guests and are focused on completing a ticket with an order on it. Ask your server next time if you are in an establishment for more traditional fare and see what happens.

Yes, some restaurants do prepare food differently for individuals. I have an allergy to wheat and so I have to have my food specially prepared. If you ask the server to prepare something specifically and they agree to do it then you should expect to receive what you ordered.
I know that Outback and Carrabas have gluten free menus that tell you how to order the foods to ensure that you are getting gluten free food.

Yes they do some Indian middle eastern Chinese also Mexican too they did not cook the same in matamoros with little rock..
