Ok, a few things come to mind. You say puff like it's cottony looking or something so I will go with that idea and rule out ick, coloration changes and breeding spots on a male.
This leave columnaris or fungus. Look closely at the spots and see if they look perfectly round and are reddish in the middle. If so that's columnaris, if not, it's a fungus
Treat the fungus with malachite green. Lots or fungus or ick medicines have this in them. Even if it's labeled an ick medication it will still kill the fungus. Treat for 10 days or for 2 days after the fungus appears to be gone, which ever is longer.
Treat columnaris with Furanace or if you can't get that go with Maracyn. Treat according to the package but for 2 weeks. Remember to remove the carbon from the filter while you treat. This treatment will also require you to cycle the tank again after you are done. Watch closely for rising ammonia levels as you treat and expect them after you treat for a few weeks.
Best of luck
MM

Male goldfish will develop white pimples and leading edges of the fins when they are about to spawn. If its 3 years or older it may happen more than once a year...
Infections can be treated with salt water in an isolation tank -- read about how to do this carefully.

if it is a "cottony" puff it is body fungus. if it is just spots of white it is ick. both are treatable with the right meds.

its called 'ick' (may not be spelled right) but they have stuff at the pet store you can buy, if you don't do it soon it will probably not be able to recover.
