Flushing is one sign, another is squirting and third is spasms of the vagina, as if vagina is milking the penis, the way cow is milked by milkman. The spasms may be at most 3-8. Wavy movement, the same way as ejaculation occurs in few spasms.

Yes if the guy is paying attention to the womans body. But not most guys pay attention or are willing to make sure a woman has an orgasm. I feel that if my woman doesn't have atleast one I am not doing my job. I know when my woman has one her face flushes and her belly and groin area can't be touched.

Yes definitly one can easily feel the jerks & movements and the facial expressions and the clutching of fingers etc all denote the orgasm and sudden rigidity and relaxation also is a sign.
