Results r what happened in the experiment.
Ur conclusion includes whether ur hypothesis was right or not, what the results of the experiment tell u, if the experiment leads u 2 want 2 conduct another 1 on something relating to that 1, etc.

results is what you get from the experiment or research. conclusion is what you conclude from the results like the following:
From these results we know that ...
Conclusion can be more than that, it can be the flaws of your project which need to be improve in the future, it can also be some new idea you come up with during the project to further strengthen the project or anything you learn from what you did.
But result is just what you expected to get when you started the project.

conclusion is what this outcome tells you. what information can you gather from this?
