It goes on the very top where it can stay fluffy and hold in the heat. It's not really a mattress even though it has that name. It's a comforter to keep you warm. A sheet doesn't keep you warm, so it goes next to your skin to protect you from the feathers that might poke through, and protect your feather mattress from B.O. .
