The most obvious difference is that egotistic is an adjective and egomaniac is a noun. Egotistic means self-centered, self-absorbed, and conceited. An egomaniac is a person who is self-centered, self-absorbed, and conceited.

It is a matter of degree. You can be an egotist(n) which means you are self-centered and self-absorbed = selfish. If you do an egotistical(adj) act you are being selfish about it. Egomaniac(n)/egomaniacal(adj) is someone who is pathologically selfish, perhaps amoral even. The suffix -ist means "being/behaving"....mania is a psychologically embeded state.

An "egotist" (noun) is a person who IS egotistic,(adverb) which is more commonly termed "egotistical", meaning that they are boastful, conceited, and think very highly of themselves.
An "egomaniac" is somone who has an obsessive, sometimes pathological pre-occupation with themselves.
(egotism on steroids, you might say!)
So it is "egotism" and "egomania" .... or "egotistic" and "egomaniacal" .... neither one is much fun to be around....
