The title inspires what I'll write about so it comes first...
Weird.

Years ago I hit upon the idea of giving them a temporary working title until the end (where I find out what the story is about). Works great.
The bad news is the only title ever think of is The Little Engine That Could.

The story flows and then the title will come. If anything you may start with what you think is a title but by the time you end it will have changed.

Yes.
Then I find it interesting to formulate the story around the title, and use elements from the title in the actual story.

Yes...then I often find the story doesn't quite fit the original title...then I change the title...then I edit the story....

Nope, because when I write, I am never sure where the story is going.
Although I have not updated it in a few days because of lack of interest, I am still working on the story I stared on my 360 blog. . .

sometimes a title leads the writer to the story, sometimes a story leads to a title

Yes, after that it is easy to draft out the plot. Having the title and naming a lead character helps to pull in other ideas/characters along the way.
