But engaged to someone else & your ex his with a cousin who is not blood related but by marriage? I still love him and I really don't know what to do? Please Help!!

50% of those fail.
go with better odds, find some who thinks the world comes to a stop when he looks at you. (that's how I've felt about my wife since I first saw her in the hall @ HS 35 years ago)

If you still love him, you have to tell him or you will be wondering 'what if' the rest of your life. If it's meant to be, you two will be together again but the only way to find out is if you are honest to yourself and let him know.

Well you should be honest with the guy that you're engaged. I don't think he deserves what you're doing.
If you don't love him what are you with him?
tell him the truth even if that means you wont be with any of them.
