Its not a metaphore. It is the story of the patient, and how deadly the disease is.

Gerard has said before that it is to shed light on the deadliness, and what people and their familes suffer from cancer.

Also, the same is with the song "Teenagers" on The Black Parade. Its to shed light on the situation.

I think it's basically just a metaphor, but remember they're telling the story about the Patient throughout the album, and he dies at a young age from cancer...
