the motor vechile sent department sent me a letter saying i would have to give up my lisence or they would suspend them my one doctor signed the form and my family doctor did not what can i do.

It depends why the family doctor failed to sign. If it was an oversight, then presumably the form could be resubmitted with the two signatures. If the family doctor failed to sign for good medical reasons, then you will simply have to surrender your licence.

You should ask your family doctor why he or she did not sign the form - perhaps there is a valid medical reason for you not to be driving!

It might just be a mistake, but without asking, you will never know.

Why is the DMV trying to take your license away? For some traffic offense? For failure to renew it? Based on some state law requirement?
