This is a complicated question.
There are different ways of using concealer. Everybody has his or her own way of doing it, and once you get practice, you can figure out your own way.
There are three keys to using concealer:
The first is to find one that is meant to conceal the KIND OF FLAW you are trying hide. (For example, green concealer is used to tone down redness, and yellow to help cover under eye circles). When purchasing concealer, try to make sure offers the kind of coverage you need. Some are heavier than others. Some are waxier. It all depends on your skin.
The second key is to PICK THE RIGHT SHADE. This can be confusing too. When picking concealer to match your skin, some people say that it is better to have concealer lighter or darker than your skintone in order to hide blemishes better. Depending on the source, it is said to use a lighter shade of concealer on pimples or darkness. I personally use one that comes as close to my skintone and undertone as possible. I don't want to highlight flaws or create greyness by using lighter or darker tones of concealer. Makeup artist Elke von Freudenberg suggests mixing your concealer and your foundation to get a good color match.
The third and in my view, most important, key to using concealer is to USE IT SPARINGLY. You don't want to slather it all over your face. It will make you look overly done and fake. Too much concealer actually accentuates the flaws on your face.
Assess how much you really need to conceal. Some women don't need concealer and just use foundation. Some women need just concealer on trouble spots and can skip the foundation. Some women need some of both. Then there are those few who are blessed with perfect skin and don't need either.
If you are just concealing minor imperfections without the use of foundation, take your pen and dab gently on the area. (Do not swipe like you are painting the fence. It will lead only to using too much and the application will most definitely be sub-par.) Then pat it in with your finger, or use a brush to gently pat it in and blend. You can then set it with some translucent powder. Alternatively, you can dispense some onto your hand and mix it with some foundation and then use a brush or your finger to pat it into your skin.
If you are using foundation and concealer, there are different ways to do it. You can apply foundation first and then apply concealer over it, or you can apply concealer first, and then follow it with the foundation.
If you choose to apply concealer first, you want to pick one that matches your skin or one that is formulated for what you are trying to cover (dark eye circles, redness, etc). When applying concealer first, it is very easy for the beginner to nitpick and to apply way too much, not realizing just how much foundation can do to even out your skintone. So when you put your foundation over it, you look like you have a mask on.
If applying concealer after foundation, make sure your concealer still looks natural once it is placed over your foundation. I actually prefer this because it's easier to see how much you really need. If you use concealer after applying foundation you'll see you didn't need as much as you thought you did initially.
As you apply concealer, you might want to hit trouble spots like eyelids or undereyes. Some people might also have some hyperpigmentation around the nostrils and mouth that they wish to conceal.
As you work, WORK IN LAYERS. You can always add more if you think you need it, but if you slather stuff on, it's a lot harder to get it off. Work in sheer, buildable layers, and your concealing job should be great.
Just remember that you don't want people looking at your concealer, you want them looking at your skin. Make sure you pick colors that complement your skin, and make sure your hands, face and brush (if you choose to use one) are clean before starting.

After applying the foundation use the concealer pen on the spots you want to cover (blemishes, little skin imperfections etc). Use fingertips to even, finish by translucent loose powder.
