I would say a gift card to somewhere like target. I just had my 2nd child and the gift cards I received have been so helpful. So she can get the items that she needs that may be worn out from the first two, or supplies that can't be reused like diapers etc. Or maybe a gift card to a resuraunt (with take-out), to make meal time easier with three kids!! I would probably wait a little while to see if she registers anywhere for the new items she needs.

I would say something she normally wouldn't spend on. Like an ultra chic diaper bag. or if she doesn't have one yet I'd get her one (or two!) of these slings I'm just obsessed with this time around, it's the peanut sling, they're so cool, the patterns are awesome (I've gotten myself one & bought one for all my pregnant friends). or also something great would be a gift certificate to a maternity store. Like Mimi maternity has the cutest clothes but are a little on the expensive side so unless it was a gift I wouldn't buy my clothes there. or I saw the coolest thing the other day, a belly cast kit! You make a cast of your pregnant belly. Great gift idea!
I'm pregnant w/ my 4th & while diapers are going to be necessary they aren't very fun to receive as gifts.

Definitely something for the mum. It's time she was pampered. When this child is born, she will really know the meaning of juggling. Perhaps a big diary or calendar so she can keep track of everything. The best present I got after my third was a week's worth of meals. And not just pasta/freezable stuff - nice stuff like stirfries or salad or homemade pizza. I'll NEVER forget that homemade pizza!

Coming from a mother of 3 and one on the way. Diapers, Diapers, Diapers. Ask her what kind she prefers and buy her a pack of each size. Or if you are close friends purchase a 30 minute massage gift certificate for her. She'll need it.

I have four, the last two are twins, & the best thing for me was diapers, diapers, diapers! Also, a box of wet wipes with a big package of refills is helpful. See if she's running low on stuff like baby wash, lotions, etc...A gift card would be nice.

Give her something that can be personalized just for that unique baby. For example, if she likes to write or scrapbook, give her a baby book such as "Baby of Mine" by Tracey Clark, that she can use to write about her baby's development every month, put photos of that child and the events that took place during its first year, etc. This is available on Amazon.com.
Or get one of those baby memory boxes that they sell at Babies R Us and other baby stores where she can store baby's mementos, such as baby books, favorite clothing and toys, etc.
There are also gift certificates for studio photos specializing in baby photography or for shadowboxes with baby's unique little hands and feet cast in porcelain (such as Wee Piggies and Paws), which you can probably find locally in your phone book.

Something TOTALLY frivolous! She most likely has everything she needs.
Some ideas of things I have done for friends with multiple children- 1. Robeez- even the unisex designs are adorable. This is my favorite shower gift. 2. Get her something just for her like a gift certificate for a manicure. Combine it with a homemade certificate offering to watch her kids while she gets the manicure.

Condoms or another method of birth control, as well as a pamphlet on how pregnancy occurs and how to prevent it, are ideal gifts for such a person. Good luck with that.
