As everyone has mentioned, the ostrich is the largest living bird in the world.

The largest extinct bird was the elephant bird Aepyornis, native to Madagascar.

It stood over ten feet high, and weighed somewhere around half a ton.

Naturally, it was flightless. It was a ratite, related to the ostrich, emu and cassowary.

No one's really sure when Aepyornis went extinct. There are shell fragments and other material that indicates that it was still around in the 10th century AD. There is even a radiocarbon date from a bone with signs of butchering that might indicate at least one bird was still alive as recently as 1880 (!!!). It was probably wiped out through habitat destruction, as its home forests were cleared and burned for farmland. There's little evidence to suggest that it was hunted to death.

The largest flying bird ever was a gigantic condor named Aiolornis that lived during the Pliocene and Pleistocene of North America. It had a wingspan up to 16 or 17 feet, and probably scavenged on the carcasses of the megafauna (mammoths and other big critters) that roamed the plains at that time.
