Go to the IRS web site, www.irs.gov.
Look on the left side of the page and click on the forms and publications link.
Then click on the form and instruction number link on the page that comes up.
Then scroll down the pdf file list until you come to 2006 Form 1040-V Payment Voucher. Click this to highlight it, then click the Review Selected Files button.
Click the link that lists your file, and a pdf version will open up. You can print whatever pages you want from there, by clicking on File in your browser menu, then selecting which pages to print.
Sorry you owe, but at least this is how to get the form.

If you are referring to just the voucher (1040-V), there is no penalty for not using the form.
Just write in the memo section of your check:
Social Security No., 2006 Form 1040.
You can either send the check with your tax return or mail it separately to the same address.

The form to make payments is the 1040V
http://www.irs.gov/pub/irs-pdf/f1040v.pdf if its for a balance due.
If its for estimated payments you need the 1040ES
