He is simply comparing the "free" government-paid (taxpayer funded) health care system to the "user-paid" premium service now becoming available.

The "paid-by-user" service is a shiny new engine with all the bells and whistles attached. Its so enormous it doesn't even fit in the firehouse.

Meanwhile, the taxpayer-funded service consists of a bucket of water.

As in ANY system where the government "pays" for health care, the Canadian system has resulted in waiting periods, rationing, and other problems that cause increasing levels of customer Dissatisfaction.

Meanwhile, those who can pay-- can pay for the best.

Its simply the Way of the World.

I don't think the truck has a flat tire. With the upgraded service you've got a firetruck to put out the "fire". With basic service, all they give you is a bucket. I think the cartoon is just showing the discrepancy in care quality between the two service options.

You need to look at the picture more closely.

Notice the bucket of water in front of the Basic Care.

Then the upgraded paid service has an actual fire truck.

Whats its saying is, the paid service gives you better treatment and care.

IE: paid service, the fire truck will come put out a fire, with basic service, you will have to put out the fire with a bucket of water.

I'd say the upgraded service has some flaws in it, aka the flat tire represents, the basic service is either unknown or non existent as nothing is in the door, only an opinion.

I think now it means faster service with the truck and the bucket means you kick the bucket expression for dying.
But not 100% sure
