slipping or tripping on loose throw rugs is a close second.

falling on stairs and slipping on ice outside

fires on stove tops, electrocution, posioning [watch them small kids around cupboards]

unlevel floors leading to standing pools of water, bad wiring, having trees to close to the home, or anything that isn't up to construction code. all of these can be hazards in a house.
