The Grammar in Use series is pretty good. It comes in different levels (basic, elementary, etc), one page is explanations and the next exercises. I have the ISBN for the Advanced version with answers: 0521498686. Some come with a CD.

Another one I like (it's more American) is The Advanced Grammar Book. Don't know the ISBN because I only have the old edition.

i aint got but one book i be thunk it had good grammor in it . it be calling it self the cat in the hat its a book done writ by a doc .
