The sonic screwdriver first appeared in the original series 5 story "Fury from the Deep", first broadcast in March 1968 and featuring Patrick Troughton as the Second Doctor. His companions at the time were Jamie McCrimmon played by Frazer Hines and Victoria Waterfield played by Deborah Watling.
Unfortunately the episodes no longer exist except for sound recordings, but a very good telesnap reconstruction of the story by the Loose Cannon team (along with a whole range of other lost Doctor Who stories) is available from http://www.recons.com/. And the best thing is its free.
The origins of the Doctor's sonic screwdriver were never explained in the series, but since the fourth Doctor's companion, Romana made herself one (she was also a Time Lord - she never referred to herself as a "Time Lady" as some fans do), it would be fair to assume that the Doctor also made his.

Doctor number 3 had the first sonic screwdriver. He got it from his TARDIS. Concept Art for the next series indicate it may be damaged or destroyed in the upcoming episode "Smith & Jones".
