Girlfriend this is an answer that you already know, kids first! If this is your first time asking to be excused than I don't see why it should be a problem. Just tell them what has to be done I couldn't imagine putting one of my children in the hospital and I'm not there I wouldn't even be able to function at work. If your boss has a problem go to Human Resource. You can get another job but you can't another little princess like the one you have! Don't ever weigh those options!

If you dont take your daughter, how is she going to get back and forth to the appointment?
Does the sugery require that she stays overnight in the hospital? I am asking so that you can anticipate some of the questions that your boss with ask of you?
If possible, do bring in proof that the surgery is going to take place when you go to your boss. Tell her how important the job is to you and ask her if you can make up the time (on saturday?) or if you can have ONE non paid day off....
Be honest (?) and tell her that you realize that this could not have come at a worse time and that you have a plan in place to care for her (her father or whomever else) after she returns home and that the job is VERY serious to you.
If she fires you this is probably NOT a company that you wanted to work for anyway!
Long after you quit or get fired from this job, your daughter will remember if she saw the smiling face of her (s)hero or not!
If you have to pick, PICK YOUR CHILD!

Since it's two weeks away, I would bring it up to your boss immediately for two reasons.
1.) It looks much better to come to your boss right away and explain how important this is, this way you seem respectful to the job, and determined to be there for your daughter, but your boss has time to make arrangements on the day you'll be gone.
2.) If he/she is a crazy non-caring loon and decides that since your still probationary this is a firing offense, you have two weeks between now and the surgery to line up another job.
Let me put it this way:
What if something happened during surgery, you say it's no big deal, but all surgery is a big deal. I could certainly never live with myself in that situation. You have to go.

WHY NOT SKED SURGERY AFTER WORK---
OR HAVE ANOTHER PERSON FILL IN FOR YOU.............
OR SPEAK TO YOUR IMMEDIATE SUPERVISOR ABOUT TAKING TWO HOURS OFF NOT A WHOLE DAY....
OR SIMPLY SIT DOWN WITH HER AND EXPLAIN YOUR SITUATION AND ASK HER ADVICE?
