Walking around your house with a stick, such as those from your closet where you hang your clothes (not sure what they are called exactly), horizontaly accross your back with your arms around it (as if you were hugging 2 people on either side) helps a LOT. I had this problem too, and it worked wonders.
Finding a big stick that will work for you might be a possible challenge, as it can't be flexible, or soft.
I made this to show you the proper way to use the stick, in case you did not understand my describtion (im kinda bad at explaining) :

It might be that your back muscles are just 'lazy'. You can also go to your local gym and ask for one of the trainers to show you a machine that can excercise your upper back muscles.
Good Luck, I really hope you get better!

We will try not to slouch off on this answer. :) If you've developed the bad habit of slouching, (which I have also), you would have to mentally break yourself of this. Maybe even something as old-fashioned as tying a string around your finger to remind you to sit up straight. Try rolling your shoulders back. You might even try sitting on a bench next to a wall to help you determine if you aren't touching the wall, and why. When you've developed the way that works best for you to do this, then keep reminding yourself to do it until it becomes a "good habit".

Do lots of situps or pilates. Spend some time at the gym and do some strength training. As the muscles in your sides strengthen, you will sit straighter and stand taller without thinking about it at all. It's not unusual to gain an inch or two in height doing Pilates. You can find Pilates on a DVD too -check your library, they probably have one.

Most people don't!
I would just say make a conscious effort to sit up straight and eventually it will become a good habit.

Just lean back into your chairs when you are sitting up, wear a corset, just in general try and be more conscious of your posture, when sitting or standing. Just keep at it and soon you'll find you do it without have to think about it!

You need to pay attention to how you are sitting and remind yourself to sit straight. sitting in a slouched position has become a habit. just keep reminding yourself you need to sit up straight.

You can buy stuff to help you sit up straight. Chairs, those stretchy things that go around your waist...ect. You can also do certain exercises to help your posture.
Good luck love!
