They just let it go. They urinate all the time in the uterus, the placenta filters it out of the fluid. Sometimes they BM inside if they are over due or in labour too long... which can cause serious problems, specially if baby breaths or swallows any as it's toxic waste. It's why when your water breaks if it isn't clear you need to worry that they let out a BM. But most of the time babies are born and then have their first BM.

what happens is the fetus takes in amniotic fluid. Which then releases (urinates) back into the amniotic fluid. It is sterile, and does no harm to mother, or fetus. A bowel movement in the fetus usually does not occur until after delivery. However, a rough labor, and stress on the baby during labor, or a pregnancy over 40 weeks, could cause the baby to have a movement in the amniotic sac, which in turn can be harmful if swallowed by the baby. Doctors will know if baby passed meconium ( baby's first bm ) in the sac. He/she will have proper equipment to suction baby's lungs immediately after birth.

Actually they do have waste. Your body takes care of it. They explained it on one of the baby delivery shows about a baby having a bowel movement in utero because the labor was lengthened and they concerned the baby would aspirate (breathe it in).

They only pee in the womb, and it just mixes back in with the amniotic fluid. I remember reading once that it was sterile, so it's not as gross as it sounds.
