Most commonly used reason is a Heart Attack, in the movies and TV show like Smallville. But in shows like the Adventures of Lois and Clark, his father is still alive along with many other comics that still have Jonathan Kent being still alive in Superman's life.

I don't remember how it happened in the books. On "Smallville" he died of a heart attack following a confrontation with Lionel Luther just after winning the election for state senate.

In the comics, I think he just got very sick and then died in his bed. I think that he died after Mrs. Kent die, which also died roughly the same way.
Clark was beside both of them when they died, he then leave Smallville.
As for how did Mr. Clark 'Superman' Kent died.
Well, I think that he was killed by Doomsday.
