It means to be conspicuous, not fit your surroundings.
A sore thumb would possibly be red and swollen. It would be obvious that it was not like the rest of the fingers and hand. It would stand out as different.

You would stand out from everyone else, you would be obviously different from the others.
When you have an injured thumb, you tend to keep it straight so it stands out away from your hand which is very noticeable.

it means "you're in the wrong joint." This is a corruption of the anatomically awkward and painful "you're joint is in wrong." Obviously referring to a sprain of the inner digits.
