In the mainstream continuity, Batman has a son with Talia, Damian.
In the Kingdom future (events after Kingdom Come), he also had a son with Talia, that had a name that translated literally to "Son of the Bat."
In both cases, the son was the heir to Ras Al Guls empire.
Batman has also took in Dick Grayson as a legal ward, and adopted Tim Drake after each lost both of their own parents (Tim was recently adopted after the loss of Tim's father in the Identity Crisis event).
In Earth-2 continuity, of the Golden Age Batman he had a daughter, Helena Wayne with Catwoman.
In the animated series, Batman Beyond (through the events seen in Justice League Unlimited) it is revealed that Amanda Waller used Batman's DNA to replace the DNA in the Sperm of Warren McGinis, so in this case Batman is the father of both Terry (Batman of the Beyond era) and Matt.
I am not sure if he had any children in any of the other elseworld tales.
I know there were some stories that took place that we about the son of Batman and the son of Superman working together, but never read any of them.

From what I remember, Batman had a son with Talia, and she faked a miscariage so they would get a divorce, and then she gave up their son.
Also, Batman's DNA was used to father Terry McGinnis in Batman Beyond, according to a Justice League episode

The only one I am aware (biologically) is Damien with the mother being Talia. He recently adopted Tim Drake, the current robin, and I believe that Dick Grayson was only ever his ward, not sure what that means. In current continuity(sp?) I believe that is all.
