I had a similar problem, and went with origami. you can try one of those links below. you make 2 boxes, one makes the top. Only thing, I used origami paper which is pretty flimsy, you might try a heavier paper.

How about you use a shoebox?
or make one with the cardboard of a shoebox..

get 5 equal pieces of wood and align them on sides nail them in leaving top side open. now get two hinges and place them at each end mark their spots nail them in and nail the other side of hinges to the sixth piece of wood. so it stays as a lid you can swing up and close.
