As a minor you need to have your parents permission. Then you need a portfolio. Which should consist of head and body shots in both colour and black and white. I would suggest a professional photographer. Then you have to send your portfolio to modeling agencies. Then you wait.
Never let the agencies ask you for money to hire you or train you that's BS

Depends on what kind of modeling you want to do. For runway modeling, you're looking to be at least 5'8 (although Kate Moss is only 5'7). If you want to do print, it doesn't really matter. For print they mostly look just for a symetrical face. Of course, if you have what it takes and just have 'the look' (like Kate Moss) there are always exceptions.
You definitely need to have what is called a "book"...basically pictures of yourself. If you can afford a photographer (usually starts at about $250 for 2 rolls of film), then great. If not, get out the digital camera and take some shots and have them printed (8x10 prints is the standard). Make you have head shots and full body, and a few changes of outfits to show how 'versatile' you are (so don't just do jeans and a t...wear a dress too).

There are no "set" requirements to be a model. Anyone can do it, you have to have a certain "look" about you and be able to photograph well. Depending on what/who you want to model for the agency or client will have specific qualities they are looking for. Victoria's Secret, Lane Bryant, Dr. Scholls, WalMart and Nike all use models and they are all very different types of people.
Good luck!!!

It depends on what kind of fashion you are going for. I am a model and I am not what you would typically see in magazines. I am only 5'3", have 34D sized breasts and a normal sized body. I do more alternitive styles, but it really depends on the direction of fashion you want to go for...

You have to be over 5’7” and stick thin (apparently these days clothes on a model are supposed to look like on a rack.)
