As you can see from the responses, it is very hard to get rid of these turf mangling creatures. There is a new product out called Talliprid, it looks like a worm, it will eliminate moles that are present and new ones that arise. It is dropped into the runs and is ingested by the mole. Only draw back is this treatment can run you about $50 and you will need at least 5 treatments a year.

My elderly neighbor planted what he called mole trees around the perimeter of his garden. The mole would eat the poisonous roots and never be seen again. I can't quite remember what they looked like, but they had purplely or reddish leaves I think, and they weren't very tall. Hope this helps!
