I think Blackburn Assurance was taken over by Swiss Life which in turn was taken over by Phoenix Life - try contacting them:

The address does not match up. Yes 151 Dale Street is at the side off the Queensway Tunnel entrance ( Also known as the Birkenhead Tunnel) but don't know what Kingsway in address stands for. Last i knew was that 151 Dale Street was occupied by Stanley Leisure.
