Like just about anything, there are exceptions to the rule. If someone were coming at me with a butcher knife, I would strike with everything I have. Forget moderation. Moderation applies when one has time to think about one's actions. Beyond that, mere instinct and reflex would be impossible to moderate. But there may be situations where an all out response would be necessary, well thought out ahead of time.

Some things require moderation, some things don't.
You can usually tell when to apply it based on how things are working out without it.
For example, marijuana - when taken in moderation it can be an effective anti-depressant, appetite stimulant, and can decrease the negative effects of glaucoma and the side-effects of radiation or chemotherapy treatment.
When not taken in moderation, it can make you into a useless, lazy stoner.
It mostly requires the applier to do a little research and determine the best course of action, and then use their best judgement. Unfortunately a lot of people aren't capable of this.

Sure. Just like food. Its great until you eat too much. Then you have obesity, illness and even death. Vitamins and medications are all about moderation. Just wish some of the far-left and far-right would practice the same!
Even more so, Islamic radicals could take a lesson in moderation!

Yes, it applies to everything.
The concept of moderation means doing as much as is necessary, but not going overboard and doing too much.
That concept can always be applied.
