It's the instructions you write after the program has already been installed and used by end users.

That would be documenting what testing went on before/after release, what bugs were found, and any release notes from QA.

Or it could be a post-mortem to go over mistakes made on a project.

I've been doing QA for a while now and haven't heard the term, btw.
