I'm trying to find examples of the effectiveness of organisations that operate with joint CEOs. Can anyone name large companies that operate with joint CEOs? How well does it work (or not)? How are the responsibilities split?

The Joint CEO concept is more common in Europe than in the USA. Airbus, for example, used this power sharing.

While I was attending a program for company presidents at the Harvard Business School, I met a Joint CEO from Germany who shared his position with 3 others. Their concept was that no one person could be equally adept at various needed areas: marketing, manufacturing, finance, and personnel management.

Currently, Mattel Australia has a Joint CEO system.

How well it works is an individual issue with each firm, as how the responsibilities are split.

The correct phrase is "Chairman" or "Managing Director"
