I bought a new set but couldn't get the old knob to come off. How much does it cost (labor) to have a plumber replace the tub faucet, control knob and shower head? Do they charge by hour or a flat fee for this?

Just the service call price. Usually $65 an hour on up. I recommend before you call, use WD40 and try to loosen it yourself! If it is corroded on try CLR or Limeaway to remove the corrosion.
That's what I would do!

How old is the faucet? Is it original to the house? Are you just trying to replace the trim, or are you replacing the whole faucet? If the faucet is over 15 years old, it is usually best to update to a newer faucet that conserves water. As far as price, there is no set price for this. To completely replace the faucet, takes about 2 1/2 hours, so I charge $150. If it is just the trim, and you supply the trim, there is a $75 service call. I hope this helps. Good luck to you!

For the handle, look for a set screw on the side of it. Not sure if you removed the cap on the end of the handle to access the screw that usually holds it on. If so, saturate a rag with vinegar and wrap it around the handle for a couple hours. This usually softens mineral deposits which form between the handle and stem. The tub spout either screws on or is held in place with a set screw located under the spout near where it meets the wall. Look for a slot in the spout and if there is one, the set screw is inside it. Shower heads just screw on. Turn it counter-clockwise while holding back on the arm. Use teflon tape to install the new one.
