I bought a new set but I couldn't get the stupid old knob to come off. How much does it cost in labor to have a plumber replace the tub faucet, control knob and shower head? Do they usually charge by hour or flat fee?

Both! ask a plumber to give you an estimate before he starts to work. Or you can pay a plumber to remove the old knobs that are stubborn. both ways!

On the whole pretty expensive - both parts and labour; it also depends on the type of gadgets you select and the alterations if any required! You can negotiate a contract price which is better than paying by hour!
