The term hand to mouth means that as soon as a person gets money in their hands it is consumed. Never to be seen again. This person does not have to be poor, they may just be very irresponsible.

Street People live a hand to mouth existence... it is said that people living in 3rd world countries where they are only making enough to feed their children and not themselves could be living hand to mouth.... basically once you get it you consume it...there is not enough to put aside for a rainy day....

It simply means existing on very little. Literally what comes in, is spent on eating for survival. It has come to mean (in the west) just having enough to pay the bills.
