I first remember it being used in reference to music and prisons. I just saw a commercial that had the line "Not ready for hard core sleeping pills?" When did perscriptions become hard core? Where does this term come from?

Here is the Oxford English Dictionary's definition:

"orig. U.S., (a) adj., denoting harsh, aggressive, or extreme versions of various types of popular music (originally punk, now also rap, techno, etc.), typically faster, louder, or more experimental than related forms, and determinedly less mainstream; (b) n., any of various forms of popular music (often a variety of an established genre) regarded as particularly extreme, aggressive, or experimental."

The first time the word appeared in print was in the Aug/Sep 1977 edition of the punk fanzine "Sniffin' Glue."

By 1981, the New York Times used the word to describe hardcore punk music as well.

Originally it was used to describe pornography but now it means anything that is done with intensity, is extremely graphic or explicit or stubbornly resistant to improvement or change

The term was coined in the second half of the 20th century to distinguish it from softcore pornography. Generally (although not always), sexual acts in softcore are simulated, while the appeal of hardcore is that the acts themselves are obviously real.
