Im coaching a tee-ball team for my senior project and I need to make up some kind of form for the parents to sign so I can put pictures on display when I do my presentation. PLEASE HELP me produce these forms!

I hereby give permission for images of my child, captured during regular and special tee-ball activities through video, photo and digital camera, to be used solely for the purposes of promotional material and publications, and waive any rights of compensation or ownership thereto.

Just type up saying that everyone knows photos taken are considered public. Don't get all wordy. Call your local YMCA or rec center, ask for a copy of theirs.
